In order to make the intake process more convenient and time-efficient for you, we have posted our paperwork forms here, so that you can print and complete them at your convenience before coming in for your first appointment.

(If you fill it out, but forget to bring it in, you will need to fill it out again when you arrive.)

New Adult Client

If you are a new adult client preparing for your first appointment with a therapist at Crossroads, please print and complete the following three forms:

2. New Client Information

3. Adult Client Self-Report

(The “Adult client self-report” is very helpful, but does not have to be completed before the first session.)
